Application Checklist for Toddlers, Beginners and Pre-K

• Apply - Applications available in Ravenna on October 1 ($95 fee)
• Complete the Parent Questionnaire.
• Submit the Teacher Evaluation Form - This form should be completed by your child’s teacher if attending a school or daycare. Optional for Beginners; required for Pre-K.
• Register for an on-campus observation - After submitting your application, register for an observation time on Saturday, January 9.
• Complete the Admission Decision Preference Form

Toddlers (2s) - Students must be 2 by August 1, 2027

Beginners (3s) - Students must be 3 by August 1, 2027

Pre-K (4s) - Students must be 4 by August 1, 2027

Application Checklist for Kindergarten - 4th Grade

• Apply - Applications available in Ravenna on October 1 ($95 fee)
• Complete the Parent Questionnaire
• Report Cards – Submit the Transcript Release Authorization Form to your child’s current school. St. Martin’s requires your child’s current-year report card (K-4th) and the final report card from the 2025–2026 school year (1st - 4th only).
• JATP – Schedule your child's JATP evaluation and submit the score report. Families may submit a psychoeducational evaluation in lieu of the JATP.
• Teacher Evaluation Form – Have your child's current teacher complete and submit this form after December 1.
• On-Campus Student Assessment – Once your application has been submitted, register for the on-campus assessment.
• Apply for Tuition Assistance – St. Martin's offers need-based tuition assistance through the Clarity platform.

*If you are applying to the Kairos Program, the JATP is not required. Instead, families should submit a current psychoeducational evaluation (within the last 3 years). This report should include the WISC-V or the WPPSI. The Kairos program is intended for students with strong intellectual potential whose academic achievement is impacted by dyslexia. Please contact the Admissions Office before submitting the application.

Application Checklist for 5th- 8th Grade

• Apply - Applications available in Ravenna on October 1 ($95 fee)
• Parent Questionnaire 
• Student Questionnaire
• 5th Grade Only - Sign JATP Authorization Form and submit JATP.
• 6th-8th Grade Only - SSAT Required *St. Martin's Code is 6735
• Principal/Counselor, Math, and English Teacher Evaluation Forms - Please submit forms to your current school after December 1.
• Report Cards - Submit the Transcript Release Authorization form to your current school. SMES requires your child’s current-year transcript as well as the completed 2025-26 and 2024-25 school-year transcripts. 
• On-Campus Student Assessment - Once your application has been submitted, please sign up for an on-campus assessment.
• Apply for Tuition Assistance - SMES offers flexible tuition payment plans and needs-based financial assistance. SMES uses the Clarity platform.

*If you are applying to Kairos support for 5th or 6th grade, families should submit a current psychoeducational evaluation (within the last 3 years). JATP is not required for 5th-grade applicants. The SSAT is required for 6th-grade applicants. The Kairos support program is intended for students with strong intellectual potential who have received prior remediation. Please contact the Admissions Office before submitting the application.
